Membership application system
ASRS is a professional society for retina specialists. I worked on two parts of their site: the membership application system, and the annual PAT survey along with the visualizations that present each year's results.

What went into it

Applications for six membership types
The application covers Practicing Retina Specialist (Regular and International), Internationally Trained Ophthalmologist, Fellow-in-Training, International Fellow-in-Training, and Ophthalmology Resident. Each type carries different requirements, so the form adapts to what someone is applying for instead of asking everyone the same questions.

Background, documentation and references
Applicants enter education, work history and prior certifications, upload supporting documentation for each, and provide references.

Payment handled in the application
The application processes payment for membership dues as part of the submission.

The annual PAT survey
I helped set up the survey form, and after the survey closed I worked with the client team to get the results out of their database.

Survey results as SVG
Built the visualizations of each year's results in SVG, in both a web version and a printable version.
